The IRB has posted new informed consent and HIPAA authorization templates on its website.
Both documents have been revised to advise participants that if an investigator leaves UAMS before completing a study, their study information may follow the investigator to his or her new institution. Note that information cannot leave the institution until the proper type of transfer agreement is in place. Research legal works with investigators to create those agreements. Other changes were also made to the consent form. A list of the changes is on page two of the template.
Please note that the IRB will start sending back contingencies reflecting these changes more or less immediately. Remember to use a fresh template document each time you draft a new consent form or HIPAA authorization form.
