The Arkansas Medical Marijuana Act of 2016 allows qualified patients to purchase and use medical marijuana from a licensed dispensary if certain criteria are met, including a written certification obtained from physician.
As a physician, You are not required to complete the certification form for your patient, however, if you determined the needs of the patient and decided to complete the form for him/her, the certification will become part of the patient’s application in getting a medical marijuana registration card, which the applicant will process it via mail, or through Arkansas Department of Health’s online application system.
By completing the Physician Written Certification, you certified that:
- You have completed an in-person patient assessment.
- The patient does have one of the qualifying conditions. (a list of qualifying conditions)
- You are licensed to practice in Arkansas.
- You have a current DEA number.
And any medical doctor or doctor of osteopathy licensed to practice in Arkansas with a current DEA number is authorized to sign the form.

For now, only the qualifying conditions will be approved for the use of medical marijuana, the Arkansas Department of Health will not accept applications that have additional write-in conditions.
