How to Apply

Here’s how to apply for a medical cannabis card.

Detailed information can be found on the Arkansas Department of Health website.

For New Patients

  • 18 years of age or older or be a minor patient with parental consent
  • Be diagnosed with a qualifying medical condition (a list of qualifying conditions)
  • Be an Arkansas resident with proof of residency. (Arkansas driver’s license or Arkansas state ID card)
  • Have the official written certification from a physician
  • Fee schedule
  • State law prohibits members of Arkansas National Guard and United States Military from obtaining a registry ID card.

Documents Needed

  • Patient application form Patient Application
  • Physician Written Certification Physician Written Certification
  • Photocopy of Arkansas issued driver’s license or state ID

For Caregivers

  • Be 21 years of age or older or be a parent or a minor patient.
  • Have not been convicted of an excluded felony offense.
  • Be an Arkansas resident with proof of residency (Arkansas driver’s license or Arkansas state ID card).
  • Fee schedule
  • State law prohibits members of Arkansas National Guard and United States Military from obtaining a registry ID card.

Documents Needed

  • Caregiver Application
  • Criminal history check (Call Toll Free 1-833-214-8619 to request a Criminal History Review Application and finger print card)
  • Photocopy of Arkansas issued driver’s license or state ID
