Laboratory Management
- Basic Sciences Instrumentation Service Request – repairs and services of research equipment (e.g., plate readers, imagers, etc.)
- Biological Safety Division – Biosafety Level 1, 2 operations, protocol forms, hazard reporting forms, biohazard waste disposal requests and more.
- Biosafety Committee (IBC)– provides biosafety information, policies, and training links for UAMS researchers.
- Chemical Safety Division – provides chemical inventory and drug spill forms, lab safety audit forms, chemical and radioactive waste disposal forms.
- Emergency Preparedness for Labs – offers guidance for the development of individual laboratory response plans.
- EHS Lab Closeout Checklist – lists activities to be completed when closing out a lab and leaving UAMS.

- Equipment Decontamination Form: Please note the new process for the disposal and movement of laboratory equipment. It requires certification as to what chemicals, biologicals or radiation were in the unit and the method of cleaning. It requires the PI to sign the form, then have someone for OHS come inspect then sign off. Then you can request Campus ops to pick it up as surplus or move the item.
