A new protocol template, dated 12.18.2024, is now available on the IRB’s “Templates, Training and Tools” webpage.
The most notable changes to this template are to beef up the various options related to HIPAA authorization waivers. There are different types of waivers, and the protocol should specify the type of waiver (or alteration) the team is requesting.
We have also highlighted the instructional text in yellow to make it easier to find (and delete).
You may also notice our other minor changes, such as deleting duplicate language and cleaning up some typos.
Remember to always start with a fresh template when writing a new protocol. We design the templates with the intent of minimizing the contingencies we need to send back before approving your study. Trying to update an old protocol for a new study will lead to missed required language that will merit contingencies.
